The first 5 years of life are very important to your child because this time sets the stage for success in school and later life. During infancy and early childhood, many experiences should be gained and many skills learned. It is important to ensure that each child’s development is progressing without problem during this period; therefore, we are inviting you to follow your child’s overall development by completing the Ages & Stages Questionnaires.
The Ages & Stages Questionnaires, Third Edition (ASQ-3) will help you keep track of your child's development. The questionnaire may be provided every 2-, 4- or 6-month period. You will be asked to answer questions about some things your child can and cannot do. The questionnaire includes questions about your child's communication, gross motor, fine motor, problem solving, and personal social skills.

The Ages & Stages Questionnaires: Social Emotional (ASQ:SE) will help you keep track of your child's social and emotional development. The questionnaire may be provided for use at 6, 12, 18, 24, 30, 36, 48, or 60 months of age. You will be asked to answer questions about some of your child's behaviors. The questionnaire includes questions about your child's ability to calm down, take direction and follow rules, communicate, perform daily activities (e.g., eating, sleeping), act independently, demonstrate feelings, and interact with others.

Please try the activities on the questionnaires with your child and record what you see and any concerns you’d like to share. Have fun completing the questionnaires with your child, and make sure he or she is rested, fed, and ready to play before you try the activities.
If the questionnaire shows that your child is developing without concerns, we will provide some activities designed for use with Ages & Stages to encourage overall development. If the questionnaire shows some possible concerns, the Guam Early Intervention System will contact you about conducting a more involved assessment for your child. All information about your child will be kept confidential.
